Submitting an Application
Where to Submit
All applications require submission of both electronic and paper forms. Reviews cannot begin until both electronic and papers forms have been received by the appropriate parties.
Tier I applicants should attach the completed form to an email with IRB in the subject line and addressed to the Chair of the IRB and the Institution Official (see right side bar). A signed paper copy of the application must be sent to the Institutional Official.
Tier II applicants should send an electronic copy and a signed paper copy to the designated coordinator for his/her department. For more information regarding Tier II applications see here.
When to Submit
Tier I Expedited, Tier I Exempt, Tier I Biological Specimens, and Tier II applications may be submitted at any time.
Tier I Full Review applications must be submitted at least two weeks prior to the next IRB meeting in order to be considered at that meeting (see right side bar for schedule).
