The Office of Facilities Management asks that all work requests be submitted on the form below.

The only requests that will be taken by phone will be emergency maintenance calls.

For Emergency Maintenance call 633-2454.

Examples of emergency maintenance calls:

An elevator that is stuck and carrying passengers

Security Risk issues
Broken glass
Doors that will not lock properly

ADA issues
Automatic doors that are not working
Handicapped stalls that are broken

Utility Interruptions
Power outages
Water leaks
HVAC system failures during bad weather
Drain backups
