(In the event you did NOT receive this via mail, you may download these forms. Parts I-IV must be filled out and turned in during orthopedic physical at Iona College.)

Important Information for Walk-On Student Athletes
The following is a list of information that you must present to the Iona College Athletic Training staff BEFORE you will be allowed to try out for a varsity team.

• Proof of a current, within the last 3 months, physical examination by a registered physician. Must include physician’s signature, phone number and date of service.
• Copy front and back of insurance card.
• Completed Iona College Physical packet, parts I-IV. (These can be downloaded above)
