Health + Wellness = Academic Success
Health Services is committed to assist students in achieving optimal wellness and academic success.
The following forms must be completed prior to orientation and the start of classes to comply with New York State Public Health Laws and Health Services requirements:
- Health Evaluation Form
- Meningitis Form
- Tuberculosis (TB) Screening Questionnaire
- Authorization for Treatment of a Minor
