Integrated Core Themes Selection Instructions

  1. Log into PeopleSoft.
  2. Choose Main Menu > Self Service > Electronic Forms > Academic Forms > ICT Selection Form.
  3. Fill in the fields and choose your school.
  4. Choose your ICT Theme Choice.

  5. Click "Next".
  6. E-sign the form by filling in the name fields.

  7. Click "Sign Electronically".
  8. You will receive the screen below to show proof that your form has been submitted. You will also have the option to view and print the PDF of the form you submitted.

Text-only Instructions

  1. Log into PeopleSoft.
  2. Choose Main Menu > Self Service > Electronic Forms > Academic Forms > ICT Selection Form.
  3. Fill in the fields and choose your school.
  4. Choose your ICT Theme Choice. The following fields will appear on the form:
    • Today's Date
    • ID
    • Last Name
    • First Name
    • School:
      • School of Arts and Science
      • School of Business
    • ICT Theme Choice:
      • Violence
      • Thomas Paine
      • Stewardship of the Earth
      • Scientific Reasoning
      • Identity: Person, Societies and Cultures
      • Feminist Interventions
      • Innocation and the Creative Mind
    You will have the option to "Save Progress" or move on by clicking "Next".
  5. Click "Next".
  6. E-sign the form by filling in the name fields.
    The page will read as follows:

    Electronic Signature

    Please read the Disclosure/Consent before you sign your form electronically.
    Typing your name exactly as it appears below signifies you are completing this form using an electronic signature. By signing electronically, you are certifying that you have read and understand the Disclosure/Consent and agree to electronically sign. You also agree to receive required disclosures or other communications to this transaction electronically.

    To continue with the electronic signature process, please enter your name and click the "Sign Electronically" button to save your information and submit your electronic signature.


    There will be two form fields, one for your first name and one for you last name.
    You will have the option to "Save Electronically" or choose "Previous" if you would like to edit the form.

    A disclaimer at the bottom of the form reads:
    If you would like to opt out of electronic signature, please click the "Opt out and print" link below to save your information and print a local copy for your signature.
  7. Click "Sign Electronically".
  8. You will have the option to view and print the PDF of the form you submitted. The screen will read as follows:
    You have successfully submitted your ICT selection.
    Thank you!

    A button will allow you to "View Form PDF."
