Club Sports Eligibility Requirements

Thank you for your interest in Club Sports! We are excited to have you as part of the team and look forward to a wonderful season ahead.

Before You Join

In order to participate, you will need to submit the following forms and information before tryouts.

A current physical from within 365 days (stamped by your doctor)
Proof of insurance (the front and back picture of insurance card)
Signed waiver

If you have yet to do so, please complete the following form and choose the sport(s) and student position(s) accordingly.

As Part of the Team

After tryouts, and if you choose to join the team, there will be a few additional forms to complete. These include, but are not limited to, the following:

Student-Athlete Concussion Facts Injury Illness and Concussions Statement
Medical Questionnaire
Student-Athlete Injury and Illness Awareness and Participation Agreement
Student-Athlete Authorization for Release of Information
Out of Network Procedures Acknowledgement Form

Additionally, we will ask that you set up an account on ImPACT and complete their at-home concussion testing assessment.
